MEDICAL EXAMINATION

PROCEDURES FOR EMPLOYEE MEDICAL EXAMINATION

  1. Download the following forms
    1. Medical Examination Forms
    2. Medical Clearance certificate
  1. Portions of the forms must be filled by Employee appropriately with the support of a qualified health care professional.
  1. The Medical examination and report must be undertaken at the KNUST Hospital or other accredited Government Health facility / Hospital.
  1. The report and certificate should bear the signature and stamp of a qualified Medical Practitioner who is registered with the Medical and Dental Council of Ghana.
  1. Informed consent is required from each employee before personal monitoring of his/her health is undertaken. As with any medical record, unless an employee gives specific permission (best given in writing) for results to be made available to the employer, the results of medical examination carried out on an employee remain confidential between the employee and the person who carried out the monitoring. The final certificate of fitness for work will be the only document submitted to the KCCR Administration for confidential storage.
  1. Employees are advised to keep a copy the Confidential Medical Report and Certificate for future reference.
